Abstract

Cerebral CT perfusion imaging is a functional study of the brain that provides physiologic information regarding cerebral blood flow (CBF), cerebral blood volume (CBV) and mean transit time (MTT). These perfusion parameters have been used in the clinical and research setting for the evaluation of patients with acute stroke, carotid occlusive disease, vasospasm, and brain tumors. The theoretical background and mathematical methods employed in the commercially available software programs are discussed. Guidelines for CT scanning protocols and recommendations for post-processing the acquired data are provided. Emphasis is directed to the clinical pearls for interpretation of CT perfusion data in determining patient management and clinical outcome, as well as the potential pitfalls that may occur when performing or interpreting CT perfusion images of the brain. The goal of this manuscript is for the reader to understand the practical theoretical, technical and clinical aspects of cerebral CT perfusion imaging.
